Congratulations to all our Lena Shaw Elementary School students that attended SFU Summer Camp. Your Tuition Passport can be redeemed by completing the application below.

The Cmolik Passport to post-secondary education is for Lena Shaw students who attended the SFU/Lena Shaw Summer Camps sponsored by The Cmolik Foundation. Credits can be used to support first-year post-secondary enrollment in an accredited university, college or trade or technical school within twenty-four months of graduation from secondary school.

Your Tuition Passport can be redeemed by completing the application below. In addition to providing us with your necessary personal information (Revenue Canada’s requirement for T4A slips), we also have some general questions as a follow-up to our program. Finally, we will need you to upload a copy of your registration and a tuition receipt to confirm your payment at a post-secondary institution. We will then mail your cheque to the address you have provided.
